As far as the distance, reference real life regional airlines to get an idea of what the "regional" range is. The big problem is that the major cities are too strong to start. Real life hub and connecting traffic are the reasons that the major airports have so many passangers. Can you make these cities slightly weaker to start, but have a larger hub effect? This would be more realistic and could could prevent runaway starts for the top players.
