I am curious to know why the airline Lufthansa in R4 is able to fly between two points outside of it's home country (Germany) considering it's a realistic world? For example I have noticed that it operates the AMS-LHR ROUTE and neither of these airports are in Germany.

There is an Openskies agreement(?) in effect for realistic worlds in most places where they are in effect in real life. EU is the most basic one I can think of that would be implemented.

As has been pointed out above:

Somebody may call their airline "Lufthansa", but they could base it anywhere in the world.
For instance, I once competed with someone called "Biman Bangladesh Airlines" who was based in London!

Additionally, after 2000, airlines based within European Union countries are able to operate flights that start and finish in EU countries other than their own, as part of an Openskies.
