It is my understanding that airlines that operate in the EU after 2000 can operate routes outside of their home country. Is it possible to extend this to:

United States-Guam-Puerto Rico

China-Hong Kong-Macao

Well, there are lots of other Openskies Agreements. eg EU-USA, but they're too hare to implement. How hard would it be to change it?

Just as a note: Openskies in the EU only counts for internal EU flights in AE

 

In regards to US-Guam-Puerto Rico, US-Guam is already implemented in AE (i.e a US airline can fly from Guam onto a 3rd country) but for some reason Puerto Rico seems to be uncooperative to extend the same thing to it, though this may have been fixed.

 

I am unaware of any China-Hong Kong-Macau Openskies agreement though?

Yuxi reported that SJU was fixed but I still cannot use it like US soil...

Well, there are lots of other Openskies Agreements. eg EU-USA, but they're too hare to implement. How hard would it be to change it?

Open skies are very handy tools ^_^ but yeah it's hardly that difficult. Possibly it's just a case of changing the status of an airport to "IN THE US" for example

Hong Kong and Macau aren't part of China in terms of air service agreements, bilaterals etc

Essentially yes. Though I've never heard of any China - Hong Kong - Macau open skies agreement and I can't recall any airline which utilises this open skies agreement if it does exist.

There was something posted before about china Taiwan flights and I can't remember the outcome but nothing was changed

Scheduled flights as well, though the bilateral is quite complicated from memory and rather difficult to implement.
