I found it very difficult to tell about the A330-200, A340-200 and Boeing 777-200ER. A330 has lower cost than the A340 while A340 can fly the same as the rest with longer runway. A340-200 is cheaper to run and longer range than A330-200, but more expensive to buy
I would go for the 777-200ER so that I could also buy the A330-300 as a MD-11 replacement.
Alternatively if long runways are ok, go A340-200 and 777-200 then -300.

Firstly, the A340-300 has the lowest fuel burn per seatmile and additionally much higher range than any but A340-200 and 777-200ER. Yet it suffers of higher purchase and maintenance costs.
Secondly, the 777-200 is pretty much on par with the A330-300, except for it's slightly higher fuel burn and higher purchase & maintenance costs. The -200ER offers the range of the A340-300 while having higher fuel burn and all the other problems of the whole 777 family (expensive).
The A330 has the advantage of having a smaller member than the 777 family while being able to fit much bigger aircraft (A345 & 6) in the same maintenance group and is the cheapest of all of them. Yet it lacks range compared to the A340 and ER models and also has worse fuel burn than the early A340s.
The 777-300 is great on the fuel burn per seatmile side if you can fill the seats, but you have to consider it's high runway usage (especially the most efficient ones are runway hogs, needing much more than 10000 ft of concrete) and it's relatively low range, which is good for TATL or continental flights within the americas or Asia, but not enough for TPAC or many Asia-Europe flights.
When considering later develpoments, the 777 is great for hauling a real lot of people over great distances while the A340/A330 family's only advantage is the earlier EIS. Both of the new A340 models are inferior to the competing Boeing in any way but runway needed (+ perhaps price).
To conclude, if you're rather focused on medium sized medium-haul aircraft and maximum flexibility, go for the Airbi, if you're looking for big longhaul aircraft, the triple 7 is great. Of course both is possible too.

I would suggest to go for mixed fleet, A330-300 and B777-200 if you need to expand quickly. Or if you prefer to have less type, then go for A330.
Contrary to what we see in real world, in AE flying longer range doesn't bring more profit. So my fleet decision usually based on capacity rather than range.

Those are F class charges, not economy. The usual starting price for a 5000 mile flight should be somewhere about $1000-1300 in economy.
